Position Summary:

Full-time position available for an Integrated Mental Health Therapist in Bellingham. Be at the forefront of a changing paradigm within health care delivery!  Full-time position on this interdisciplinary team.  Responsibilities include assessment, brief intervention, consultations with primary care, patient-centered health action plans, care coordination and care transitions for Adults and Children.  You will be part of a team that will work toward better health outcomes by applying evidence based practices.  This is a dynamic program oriented toward health care transformation.
