POSITION SUMMARY: Under the supervision of the Engagement Services Supervisor, the Case Manager provides support, intervention, and referral services to young people who we encounter in order to help them remove barriers to independence and stable housing.

**COVID vaccinations are mandated at YouthCare. YouthCare will reasonably accommodate those individuals with medical conditions and/or religious beliefs that prevent them from obtaining the vaccine. 


  • Maintain a caseload of approximately 20 young people who are receiving services at South Seattle Shelter (Jackson st).  and assist them in removing barriers to independence through engagement in education, employment, and progressive goal setting
  • Provide a minimum of 20 hours weekly direct contact with young people on the case management caseload including face-to-face counseling, advocacy and transportation and assisting young people in meeting their basic needs of food, housing and safety.
  • Link young people to opportunities that allow them to gain skills to become successful adults.
  • Act as a mentor and advocate for young people participants.
  • Engage young people through Motivational Interviewing, Trauma Informed Care, Harm Reduction, and strengths-based approaches.
  • Ensure all client-related data is both current and accurate in our Case Worthy database System.
  • Ensure that all the clients in your caseload have active goal plan and update progressive case note for each client.
  • Complete and submit all documentation and grant reporting in a timely manner.
  • Lead weekly client meetings discussing upcoming employment and service opportunities.
  • Participate in weekly or bi-monthly supervision/team meetings
  • Engage young people in educational facilities and service centers south of Seattle.
  • Promote YouthCare programs to young people and community partners.
  • Build trust and rapport with young people experiencing homelessnessyoung people who have experienced sexual exploitation and/or young people who are vulnerable.
  • Encourage young peoples’ movement from emergency services to shelter, long term case management, education and employment programming.
  • Transport young people to appointments
  • Provide de-escalation, crisis intervention and basic counseling services.
  • Multi-task in a fast-paced and hectic environment.
  • Develop relationships with other community agencies to facilitate contacts and services for young people.
  • Refer young people to appropriate and necessary services.
  • Participate in a weekly provider coordinate entry (CEA) meeting organized by the city
  • Attend grant funder sponsored meetings as required for position.
  • Submit a biweekly case status report to the Supervisor
  • Spend time working collaboratively with the Community Outreach Team participating in outreach activities and working in high traffic areas frequented by young people experiencing homelessness to establish relationships and move individuals into shelter.
